E insiste que o motivo é "porque não é agradável olhar para dentro de nós próprios" e porque os médicos preferem "atacar as causas externas pela profilaxia, a desinfecção e assim por diante", em vez de enfrentar as causas verdadeiras, que são internas. Numa formulação mais recente de Karl Menninger: "a doença é em parte aquilo que o mundo fez à vitima, mas numa parte mais significativa aquilo que a vítima fez do mundo e de si própria..."Opiniões tão absurdas e perigosas acabam por fazer pesar sobre o paciente a responsabilidade da doença e não só enfraquecer a capacidade do paciente para compreender o alcance de um tratamento médico razoável, como também, implicitamente, desviá-lo de tal tratamento. A cura dependeria sobretudo das capacidades de auto-estima do paciente, já duramente postas à prova ou enfraquecidas.
